Flu Vaccination Requirement

Wake Forest students are required to get a flu shot and submit documentation by December 15, 2021. Learn about policy details at go.wfu.edu/flupolicy2021.

Once you’ve received a flu shot, you’ll need to upload the shot documentation into your Student Health Portal.

*Note: if you receive your flu vaccine at the on-campus clinic, you documentation will be automatically uploaded to your student health portal as long as as long as you sign your name in the consent field. If you do not complete the consent field, you will have to upload it yourself (see below).

Submitting Compliance Documentation

  1. Login to the Student Health Portal using your WFU credentials. If you’re unable to login to the portal or load the page, try connecting to WFU VPN.
  2. Click Document Upload at the top.
  3. Choose Documentation of Influenza Vaccination from the drop down menu.
  4. Attach a clear photo of the shot documentation that was given when you received the shot. Your documentation must include your name, the vaccine you received, the date you received it, and the Pharmacist’s signature.
  5. Hit Save.

You can check your Student Health Portal under immunizations for confirmation that we received your flu vaccine documentation.

Request for Exemption

  • If you were approved for a religious exemption for the influenza vaccine for academic year 2020-21, that exemption carries over for 2021-22; no additional action is required on your part.
  • If you were approved for a medical exemption for the influenza vaccine for academic year 2020-21, please call the Student Health Service at 336.758.5218 to find out if your exemption is carried over to the 2021-22 year.
  • If you were approved for a medical OR religious exemption for the COVID vaccine for academic year 2021-22, that does NOT carry over automatically for the influenza vaccine. You will have to submit a separate medical or religious exemption form for the flu vaccine.
  • For new religious or medical exemption requests: you must fill out a medical or religious exemption form. Note that a medical exemption request requires your doctor to provide a detailed explanation of why you should be exempt; please give your doctor’s office sufficient time to complete the form. Religious exemption requests must include your signature.

  • What if I didn't receive documentation with my flu shot?

    If you received a flu shot from CVS, you can get your flu shot documentation reprinted at any location by giving them your name, date of birth, and a photo ID. 

    If you received your shot from your primary care physician or another pharmacy, contact them directly about providing your documentation. Tip: if you received your vaccine at a chain drugstore/pharmacysuch as Walgreens or an urgent careit’s best to call the specific branch you visited to request documentation.

    Your documentation must include your name, the vaccine you received, the date you received it, and the Pharmacist’s signature.

  • Is the flu vaccination mandatory?

    The flu vaccination is mandatory for all students again this year. Students are required to get the flu shot and submit proof of the vaccination to Student Health Service by December 15, 2021.

  • How do I request a medical exemption from the flu shot?

    Individuals with documented medical contraindications to influenza vaccine are exempt from this policy. This would include a history of severe, life-threatening allergies to influenza vaccine or any ingredient in the vaccine documented by a medical provider. This might include gelatin, antibiotics, or other ingredients. See more details from the CDC.

    Students requesting a medical exemption must download the form below and have your medical provider complete it. Upload a copy of the completed form to the document upload section of your Student Health Portal by December 15, 2021.

    Note: if you were approved for a medical exemption from the influenza vaccine last year, contact Student Health Service at 336.758.5218 to learn if you need to request again this year.

  • How do I request a religious exemption from the flu shot?

    Students with bona fide religious beliefs contrary to immunization with the influenza vaccine are exempt from this requirement.

    • Documentation of bona fide religious beliefs should complete the Influenza Vaccine Exemption Request Form and uploaded to the document upload section of your Student Health Portal by December 15, 2021. Note: if you were approved for a religious exemption from the influenza vaccine last year, you do not have to re-submit the exemption form for this year.
