Mandatory Flu Vaccinations
All members of our community are required to get a flu shot by December 15, 2020 and will be required to submit proof to Student Health prior to returning to campus for Spring 2021. Learn about policy details.
Uploading Compliance Documentation
Once you’ve received a flu shot, you’ll need to upload the shot documentation into your Student Health Portal.
- Log in to the portal.
- Click “Document Upload” at the top.
- Choose “Documentation of Influenza Vaccination” from the drop down menu.
- Attach a clear photo of the shot documentation that was given when you received the shot.
- Hit “Save”.
You’ll get a confirmation email once your submission is received. Student Health will contact you if there’s a question with your submission.

Students who are not taking courses, participating in activities, or working on or at any Wake Forest University campus or property are exempt from this policy.
Individuals with documented medical contraindications to influenza vaccine are exempt from this policy. This would include a history of severe, life-threatening allergies to influenza vaccine or any ingredient in the vaccine documented by a medical provider. This might include gelatin, antibiotics, or other ingredients. See more details from the CDC.
Students requesting a medical exemption must download the below form and have your medical provider complete it. Upload a copy of the completed form to the document upload section of your Student Health Portal.

Students with bona fide religious beliefs contrary to immunization with the influenza vaccine are exempt from this requirement. Documentation of bona fide religious beliefs should be in writing and uploaded to the document upload section of your Student Health Portal.
