Mandatory Flu Vaccinations

All members of our community are required to get a flu shot and submit documentation by December 15, 2020. Learn about policy details. You can fulfill this requirement at your doctor’s office, a pharmacy, or at Student Health Service.

Students who are not taking courses, participating in activities, or working on or at any Wake Forest University campus or property are exempt from this policy.

Submitting Compliance Documentation

Once you’ve received a flu shot, you’ll need to upload the shot documentation into your Student Health Portal.

  1. Login to the Student Health Portal.
  2. Click “Document Upload” at the top.
  3. Choose “Documentation of Influenza Vaccination” from the drop down menu.
  4. Attach a clear photo of the shot documentation that was given when you received the shot. Your documentation must include your name, the vaccine you received, the date you received it, and the Pharmacist’s signature.
  5. Hit “Save”.

You’ll get a confirmation email once your submission is received. Student Health will contact you if there’s a question with your submission.

  • Who does this policy apply to?

    Any undergraduate, graduate, professional student, or employee of Wake Forest University that will be visiting a Wake Forest campus in the Spring 2021 term (Reynolda, Brookstown, Wake Downtown, Wake Washington, and Charlotte campuses.) will be required to submit flu shot documentation to Student Health Service.

    The only members of our community that are exempt from this requirement are those who are:

    • Not taking courses, participating in activities, or working on or at any Wake Forest University campus or property.
    • Students and employees who qualify for medical exemptions.
    • Students and employees who qualify for religious exemptions.
  • What should I do if I am missing or have incomplete documentation from the student flu shot clinic?

    If you received your flu shot at the Student Flu Shot Clinic held on campus, and have misplaced or were given incomplete documentation you can get a new slip at any CVS location. To get another copy you’ll be asked to provide your name, date of birth, and a photo ID.

    Any CVS can reprint this documentation for you, however the closest CVS to campus is:

    The documentation you submit to Student Health Service must have your name, the vaccine you received, the date you received it, and the Pharmacist’s signature.

  • What if I didn't receive documentation with my flu shot?

    If you received a flu shot from CVS, you can get your flu shot documentation reprinted at any location by giving them your name, date of birth, and a photo ID. 

    If you received your shot from your primary care physician or another pharmacy, contact them directly about providing your documentation. Tip: if you received your vaccine at a chain drugstore/pharmacysuch as Walgreens or an urgent careit’s best to call the specific branch you visited to request documentation.

  • Will I receive any paperwork proving I received the flu shot?

    Every student who receives their shot should receive a paper form with proof of vaccination. Please keep this paper copy, as you will need to submit a photo of it to Student Health Service as proof of your compliance with the flu shot policy.

  • Is the flu vaccination mandatory?

    The flu vaccination is mandatory for all students this year. Students are required to get the flu shot and submit proof of the vaccination to Student Health Service by December 15, 2020.

  • How do I request a medical exemption from the flu shot?

    Individuals with documented medical contraindications to influenza vaccine are exempt from this policy. This would include a history of severe, life-threatening allergies to influenza vaccine or any ingredient in the vaccine documented by a medical provider. This might include gelatin, antibiotics, or other ingredients. See more details from the CDC.

  • How do I request a religious exemption from the flu shot?

    Students with bona fide religious beliefs contrary to immunization with the influenza vaccine are exempt from this requirement. Documentation of bona fide religious beliefs should be in writing and uploaded to the document upload section of your Student Health Portal.

  • Where can I get my flu shot?

    Student Health Service will offer the flu shot in their offices throughout the semester. Students will need to make an appointment to receive their flu shot at Student Health by emailing apptshs@wfu.edu (recommended) or calling 336.758.5218. Insurance is not accepted for the flu shot at Student Health. The cost is $30, which will be billed to their student account.

    Students can opt to get their flu shot off campus through a pharmacy or doctor’s office. They will need to get the shot and submit proof of vaccination to Student Health Service by December 15, 2020.
